Alcohol and Energy Drinks: What the Research Actually Says

If you are wondering whether a vodka Red Bull is meaningfully riskier than the same vodka with plain water, here is the short answer: the peer-reviewed work on alcohol and energy drinks is messier than the headlines suggest, but it converges on one point — caffeine can change how awake you feel without changing how impaired you are. That gap is the risk, and it is why Canada regulates pre-mixed caffeinated alcoholic drinks so tightly.

What the peer-reviewed work on alcohol and energy drinks actually found

The concern has a name: masked intoxication, or "wide awake drunk." The theory is that caffeine suppresses the sedative cues that normally tell you to stop, so you keep drinking, misjudge your state, and take risks you otherwise would not.

The evidence that supports the concern

The most cited early survey is O'Brien and colleagues (2008), a web survey of 4,271 students across 10 North Carolina universities. Among past-30-day drinkers, 24% reported mixing caffeine and alcohol this way. Those students averaged 6.4 heavy episodic drinking days versus 3.4 (p < 0.001), about twice as many weekly episodes of drunkenness (1.4 versus 0.73 days, p < 0.001), and higher rates of injury, needing medical treatment and riding with an intoxicated driver — differences that persisted after adjusting for how much they drank.

The closest thing to a verdict on harm is Canadian. Roemer and Stockwell (2017), at the University of Victoria, reviewed the literature under PRISMA guidelines and concluded that the results "support the association between AmED and increased risk of injury" — while adding that "substantial variability in harm outcomes and methodology makes it difficult to determine the extent of this risk."

On mechanism, Marczinski and colleagues (2012) dosed 18 participants across four conditions. Alcohol slowed dual-task information processing and impaired motor coordination, and adding an energy drink did not reduce that impairment at all. What changed was subjective: less mental fatigue, more stimulation. That is the caffeine and alcohol problem in one sentence — the impairment stays, the feeling of being impaired softens.

The evidence that cuts against it

Here is the part most articles skip. Several careful studies looked for masked intoxication and did not find it.

  • Verster et al. (2015) breathalysed 997 people leaving bars in Utrecht over five nights. Breath alcohol was identical across groups (0.074% with energy drinks versus 0.074% without, p = 0.94), as were subjective intoxication ratings (4.5 versus 4.6 on a 10-point scale, p = 0.96). Energy drink use did not predict how drunk people said they felt.
  • Ulbrich et al. (2013) ran a randomised, double-blind, four-treatment crossover trial in 52 healthy men using 46.5 g of ethanol as vodka. Across 18 visual-analogue items, no significant masking effect appeared for caffeine or energy drink.
  • Alford et al. (2012) tested 20 volunteers at two alcohol doses double-blind and found "no evidence that the energy drink masked the subjective effects of alcohol at either dose."

A 2018 meta-analysis by Verster and colleagues pooled six experimental studies and found essentially no effect on subjective intoxication (ES = −0.021, 95% CI [−0.310, 0.268], p = .747). People who mix did drink more overall than those who do not (ES = 0.535, 95% CI [0.378, 0.692], p = .0001) — but within the same individuals, energy-drink nights were not heavier. The drinkers differ, not necessarily the drinks. That review was funded by Red Bull GmbH, which the authors disclose.

Why Health Canada restricts pre-mixed caffeinated alcoholic beverages

Canadian regulators did not wait for the science to settle. The position is blunt, and it is stated in the Canada Gazette: "It is illegal in Canada to sell alcoholic beverages that contain caffeine as an additive."

A Health Canada advisory dated 27 September 2018 repeats that "in Canada, alcoholic drinks that have directly added caffeine are illegal" and warns that mixing caffeinated drinks with alcohol can contribute to alcohol poisoning, dehydration, and alcohol-related injury or death.

The same instinct produced the flavoured purified alcohol rules registered 22 May 2019, capping single-serve containers of 1,000 mL or less at 25.6 mL of alcohol — described in the regulation as 1.5 standard drinks, putting a Canadian standard drink at about 17 mL of pure alcohol.

What the rules mean on a Canadian shelf today

Three things hold for canned cocktails Canada-wide right now:

  • No legally sold alcoholic beverage in Canada has caffeine added to it. Trace caffeine can still arrive through flavouring ingredients such as guarana, coffee or chocolate, or in a pre-mixed rum-and-cola where the cola is itself a caffeinated soft drink.
  • Energy drinks must carry the warning. Health Canada's guidance requires the caution "Do not mix with alcohol" on caffeinated energy drink labels, alongside "Not recommended for children, pregnant or breastfeeding women and individuals sensitive to caffeine," and caps single-serve containers at 180 mg of caffeine.
  • The mixing happens at the bar, not the plant. Essentially every vodka Red Bull in Canada is poured by hand, so the dose is unlabelled and unmeasured.

What to actually do tonight

  • Track drinks, not feelings. Impairment is set by the alcohol; how alert you feel is a poor proxy for it. Count standard drinks.
  • Never treat caffeine as a sobering agent. Nothing in this literature shows caffeine reduces alcohol-induced impairment of reaction time or motor coordination. Only time lowers blood alcohol.
  • Use pre-portioned formats if you lose count. A sealed can with a printed ABV is easier to tally than a free pour. That is a measurement advantage, not a health benefit.
  • Anchor to a number. Canada's Guidance on Alcohol and Health (17 January 2023) sets a continuum: 1–2 standard drinks a week is low risk, 3–6 moderate, 7 or more increasingly high, with no more than 2 on any occasion.
  • Separate the caffeine from the drinking. If you want the energy drink, have it well before, not alongside.

What this research does not show

The limits are real:

  • The lab studies are small — 18, 20 and 52 participants. Small samples miss real effects.
  • Survey findings such as O'Brien's are correlational. People who choose vodka Red Bull may already be heavier or more risk-tolerant drinkers, and adjustment does not remove that.
  • Masking has repeatedly failed to replicate under controlled conditions, even though the injury association keeps appearing in field data. Those two facts remain unreconciled.
  • Funding matters. The largest meta-analysis was industry-supported; the alarm-raising surveys were never designed to establish causation.
