If you have a drink to help you wind down and then wake at 3 a.m. feeling wired, the research on alcohol and sleep explains it. A nightcap can help you fall asleep, but it changes how the rest of the night unfolds. REM sleep is hit first. In the lab, the effect shows up at doses many Canadians would call "just a couple," and no study has pinned down a safe cut-off time before bed. Here are the numbers from sleep labs, which record brain activity overnight, rather than from how people say they slept.
Alcohol and sleep: the short answer
The largest recent synthesis is a 2025 systematic review and meta-analysis in Sleep Medicine Reviews by Gardiner and colleagues, which pooled 27 studies of healthy adults. Its main findings:
- REM sleep is delayed and shortened. Across the studies that reported minutes, alcohol cut REM sleep by 11.3 minutes (95% CI −20.8 to −1.8; 13 effect sizes, 96 participants, p = 0.026). REM's share of the night fell by 2.8 percentage points (p < 0.001).
- The effect grows with the dose. For every additional 1 g of alcohol per kg of body weight, REM fell by a further 40.4 minutes (p = 0.009).
- Falling asleep faster takes a large dose. Sleep came on significantly faster only at about 0.85 g/kg, and deep sleep (N3) started sooner only at about 0.95 g/kg.
- Total sleep time was not clearly changed. The pooled figure was −10.1 minutes but not statistically significant (p = 0.160), with a very wide prediction interval.
How a nightcap reshapes REM and slow-wave sleep
An earlier qualitative review of every known study in healthy volunteers, by Ebrahim, Shapiro, Williams and Fenwick (2013), found a two-part pattern. At every dose, people fell asleep faster, slept more solidly in the first half of the night and were more disrupted in the second half. The most consistent effect on REM was a delayed first REM period at all doses. Most studies also found more slow-wave (deep) sleep in the first half of the night, and whole-night slow-wave sleep rose at high doses.
The Gardiner review adds numbers to that picture. In two studies that split the night, deep sleep rose by 6.4 to 9.6 minutes in the first three hours. In the second half, deep sleep fell by 5.2 to 6.4 minutes and REM fell by 11.0 to 17.4 minutes. The authors note that the second half of the night is normally REM-heavy, and that is when alcohol's sedative effect wears off and its breakdown products build up. Some people expect REM to rebound later in the night and make up the loss. The pooled data suggest total REM still ends up lower.
The dose at which effects appear
Studies measure dose in grams of alcohol per kilogram of body weight. The Gardiner model puts these thresholds on it:
- About 0.35 g/kg: the first REM period starts later.
- About 0.50 g/kg: total REM time falls significantly.
- About 0.85 g/kg: you fall asleep significantly faster.
- About 0.95 g/kg: deep sleep starts sooner.
The paper converts these using a 10 g "standard drink" and a 62 kg body. It describes 0.50 g/kg as "approximately two" standard drinks in one place and "approximately three" in another. Canadian drinks are bigger: Health Canada defines a standard drink as 13.45 g of pure alcohol, such as a 341 mL beer at 5% or a 43 mL shot of 40% spirits. By our arithmetic, for a 70 kg adult, 0.35 g/kg is about 1.8 Canadian standard drinks, 0.50 g/kg about 2.6, and 0.85 g/kg about 4.4. In plain terms, the REM effect starts at around two to three drinks. The faster-to-sleep effect people want from a nightcap needs roughly twice that, and a bigger dose means a bigger REM loss.
Drinking before bed: when does the disruption fade?
This is the question readers ask most, and the evidence is weakest here. The Gardiner authors say most studies gave alcohol within about three hours of bedtime, so they "could not establish" how timing matters. Their model estimated only 0.06 more minutes of REM for each extra hour between drinking and bed, which is too small to be useful.
A small study that tested an earlier drink is not reassuring. Landolt and colleagues (1996) gave 10 healthy men (average age 61.6) 0.55 g/kg of alcohol six hours before bedtime. By lights-out, breath alcohol was zero in every participant. Compared with a mineral-water night, total sleep time, sleep efficiency and REM sleep were all lower, and time awake in the second half of the night doubled. The men also rated their sleep as lighter.
So no study has established a "stop drinking X hours before bed" rule. One small study found effects six hours out, even with no alcohol left on the breath.
Alcohol REM sleep effects: what labs measured vs. what people report
Here, what people report and what the machines record don't match. The Gardiner review found no clear effect of alcohol on how long people thought they slept, how long they thought they lay awake, or how well they rated their sleep. The most common subjective finding was that people felt they fell asleep faster, but even that was inconsistent. Meanwhile, the overnight recordings showed REM loss. Because the drowsiness is easy to notice and the REM loss isn't, a nightcap can feel like it helped.
Wearable data points the same way. In an observational study of 4,098 Finnish employees, Pietilä and colleagues (2018) compared each person's nights with and without alcohol, using heart-rate variability from the first three hours of sleep. A heart-rate-variability "recovery" score dropped by 9.3, 24.0 and 39.2 percentage units after low (≤0.25 g/kg), moderate and high intake, and the effect was significant even at low intake (p < .001).
What this means for your nightcap sleep quality tonight
- Don't use alcohol as a sleep aid. The Gardiner authors conclude that any help falling asleep is "likely outweighed" by the REM disruption.
- Count in grams, not glasses. Around two Canadian standard drinks is where lab studies start to see REM changes for many adults. Smaller bodies reach the threshold sooner.
- Finish earlier, but don't count on the clock. More time before bed probably helps, but the six-hour study shows it isn't a guarantee.
- Eat with your drink. The review notes that drinking on a full stomach slows the rise in blood alcohol.
- Protect important nights. Before an exam, a big meeting or an early start, the best choice is to skip the drink. Canada's Guidance on Alcohol and Health lists "better sleep" among the benefits of not drinking.
Caveats: what the research does not show
- Gardiner and colleagues judged 21 of their 27 studies to be at risk of bias, and the samples were small.
- The results on total sleep time, sleep efficiency and time awake after falling asleep were too uncertain to call either way.
- Twelve of the 27 studies enrolled only men and four enrolled only women, so the review could not work out sex differences, and it excluded people with sleep disorders and adults over 70.
- The Ebrahim review was publicly challenged in a 2015 commentary arguing its design and statistics could not support its conclusions, and the authors published a rebuttal.
- The timing evidence rests largely on one 10-person study.
- The Finnish study relied on self-reported drinks, measured heart rhythm rather than sleep stages, and two of its authors worked for the wearable company.
Most importantly, none of this research tested any particular brand. A still vodka water like Aura has no bubbles, which some people find easier on the stomach. But the alcohol in the can is the same alcohol, and it affects sleep the same way. Aura is not healthier, not less intoxicating and not a sleep aid.
