In the water aisle of a Canadian grocery store, one bottle costs three times the one beside it. The only visible difference is a number: pH 9.5. Ask what does pH mean water shoppers should pay more for and you get confident language about acidity, detoxing and cellular hydration, mostly unsourced. The usable answer: pH describes how intensely acidic or basic a liquid is at the moment you measure. It says little about how much mineral is dissolved in the bottle — and mineral content is the part that has actually been studied.
What does pH mean water chemists would actually recognise?
pH measures the acidity or basicity of water on a logarithmic scale from 0 to 14, with 7 as neutral. Logarithmic does the heavy lifting: each whole unit is a tenfold change in hydrogen ion activity, so pH 9 water has one tenth the hydrogen ion activity of pH 8, not "one unit less."
The World Health Organization's fact sheet in its Guidelines for Drinking-water Quality states flatly: "No health-based guideline value is proposed for pH. Although pH usually has no direct impact on consumers, it is one of the most important operational water quality parameters." It adds that pH is "not of health concern at levels found in drinking-water."
In Canada, Health Canada sets an operational guideline of pH 7.0 to 10.5 for finished drinking water. Note the word operational: the range keeps treatment effective and controls corrosion in the distribution system. It is not a claim that 8.5 beats 7.5 for you — and it means plenty of Canadian tap water is already alkaline before it reaches your glass.
Intensity versus quantity: why mineral content in drinking water matters more
Think of pH as the pitch of a note and buffering capacity as its volume. pH gives the intensity of the acidity right now. Buffering capacity — driven mostly by dissolved bicarbonate and minerals — is how much acid the water can absorb before its pH moves at all. A water can read pH 9 on a strip and contain almost nothing capable of neutralising anything.
Koufman and Johnston tested an artesian well water with natural bicarbonate at pH 8.8 and reported in Annals of Otology, Rhinology & Laryngology (2012) that its hydrochloric acid-buffering capacity "far exceeded that of the conventional-pH waters" — same pH territory as many bottled products, very different chemistry. When you compare labels, the mineral content drinking water figures are the informative ones: milligrams per litre of calcium, magnesium and bicarbonate, or total dissolved solids.
What the alkaline water studies evidence actually shows
Bone turnover: a bicarbonate result, not a pH result
Wynn and colleagues ran a controlled comparison in Bone (2009). Thirty young women, mean age 26.3, ate an identical weighed diet supplying 965 mg of calcium daily and drank 1.5 L/day of one of two waters for four weeks: an acidic, calcium-rich water (520 mg calcium and 291 mg bicarbonate per litre) or an alkaline, bicarbonate-rich water (547 mg calcium and 2172 mg bicarbonate per litre). Parathyroid hormone fell with the alkaline water (p=0.022), as did serum C-telopeptide, a bone resorption marker (p=0.023). The acid water produced no effect.
Read the numbers, not the headline. Calcium was nearly identical; what differed was bicarbonate, 2172 mg/L against 291 mg/L. That is a heavily mineralised mineral water, not a lightly mineralised bottle that reads pH 9 — and a blood marker moving over four weeks in 30 people is not a fracture outcome.
Post-exercise hydration markers
Weidman and colleagues ran a randomised, double-blind, parallel-arm trial in 100 healthy adults (50 male, 50 female, aged 31 ± 6) in the Journal of the International Society of Sports Nutrition (2016). Participants exercised in a warm environment until they lost 2.0 ± 0.2% of body weight, then rehydrated over 120 minutes. High-shear systolic blood viscosity fell 6.30% with alkaline water versus 3.36% with control (p=0.03). The diastolic comparison (9.35% versus 5.43%) missed significance (p=0.074), and plasma osmolality, bioimpedance and body mass showed no significant differences.
The funding statement belongs in the same breath: the study "was supported by a grant from Essentia Water, and alkaline bottled water for the study was provided by Essentia Water," and one author reported consulting fees and stock options from that company. One industry-funded trial on a surrogate marker is a starting point, not a conclusion.
Where the evidence runs out
- The biggest claims have the least behind them. Fenton and Huang screened 8,278 citations and 252 abstracts for BMJ Open in 2016 and found one eligible study on dietary acid load, alkaline water and cancer — a cohort showing no significant link to bladder cancer (RR 1.15, 95% CI 0.86–1.55, p=0.36). Their conclusion: "there is almost no actual research to either support or disprove these ideas," and promoting alkaline water for cancer prevention or treatment "is not justified."
- Against ordinary mineral water, differences mostly vanish. A 2024 systematic review of 10 studies in Reviews on Environmental Health found alkaline and oxygenated water "did not show any significant difference on gut microbiota, urine pH, blood parameter, or fitness parameter" compared with mineral water.
- The Koufman finding is in vitro. Pepsin was inactivated in a laboratory dish, not in patients, and no symptom outcomes were measured — yet it is cited as though it were a clinical trial.
- The literature is small and short. Samples of 30 to 100, durations of weeks, surrogate endpoints, little replication, and in several cases funding from companies selling the product.
Be clear about what none of it shows. Alkaline water does not meaningfully change your blood pH, which your body regulates tightly regardless of what you drink. It does not prevent, treat or cure disease. And it does not make an alcoholic drink healthier, safer or less intoxicating.
How to read a water label in Canada today
- Treat a bare pH number with no mineral figures as marketing, not information.
- Look instead for milligrams per litre of calcium, magnesium and bicarbonate, or total dissolved solids — that is what the bone research was about.
- Check your own tap water first: most Canadian municipalities publish an annual water quality report listing pH and hardness, and many already sit above neutral.
- If it tastes better and you drink more water because of it, that is a legitimate reason to buy it — and the only one the evidence comfortably supports. Never substitute it for medical care or medication.
